Living with rheumatoid arthritis (RA)  can feel like a constant battle happening inside your own body. The stiffness, swelling, and fatigue can make even simple daily tasks challenging. If you’ve been searching for a treatment that helps break the cycle of inflammation and gives you a chance to regain control of your daily life, Orencia infusion therapy may be a solution worth considering. You receive the Orencia infusion therapy treatment in a comfortable, supportive environment with a team dedicated to helping you achieve long-term relief.

This guide explains how Orencia works, what to expect during an infusion, and how it may support your long-term RA management plan.

What Orencia Is and How It Works

Orencia (abatacept) is a biologic disease-modifying antirheumatic drug (DMARD) that targets one of the key drivers of rheumatoid arthritis: continuous T-cell activation. In RA, your immune system attacks your joints, causing erosiveinflammation that can damage cartilage and bone.

Orencia interrupts the immune response by blocking  T-cell activation, which can prevent the signals that trigger swelling, pain, and stiffness. Over time, Orencia can reduce harmful immune activity, easing symptoms and helping protect your joints from progressive damage. Because it also lowers your ability to fight infection, you should watch for symptoms and contact your provider promptly if you develop fever, chills, sweats, persistent cough, sudden and unexplained fatigue.

For patients who have not experienced adequate reduction in symptoms and/or disease control from other RA medications, Orencia offers a more targeted approach that works at a deeper immune-system level.

What to Expect During Your Orencia Infusion

Patient comfort and safety are always the priority. Orencia IV infusions follow a standard schedule: You will start with an infusion at weeks 0, 2, and 4, followed by maintenance infusions once every four weeks. Each infusion typically takes about 30 minutes, making Orencia one of the more time-efficient infusion therapies available for RA. Plan a total of 1 hour for your whole appointment.

During your appointment:

  • You may relax in a comfortable recliner while your medication is administered.
  • Your nurse will monitor you throughout the process for any signs of adverse reactions and to meet any needs you may have during your infusion.
  • Most patients spend time reading, listening to a podcast, enjoying TV time, or simply resting.

When the infusion is complete, you may return to your normal daily activities without any downtime.

Possible Side Effects to Be Aware Of

Many patients tolerate treatment well; it is not unusual to feel some negative side effects for 1 to 3 days after Orencia infusions. Common side effects can include:

  • eadache
  • Upper respiratory tract infection
  • Nausea
  • Sore throat
  • Fatigue

Discuss with your provider if these symptoms persist or worsen. More serious risks, while uncommon, include:

  • Increased risk of serious infections,  e.g., pneumonia and sepsis.
  • Severe Infusion reactions (uncontrolled fever, or sudden and extreme shortness of breath)

Before starting Orencia, you will need screening for tuberculosis and hepatitis B, and complete treatment if results are positive. You must have a negative result for both Hep B and TB before starting Orencia.  Patients with chronic obstructive pulmonary disease (COPD) have a higher risk of respiratory complications and require close monitoring by their ordering provider.

Your provider will order regular blood tests to monitor for signs of immune suppression, worsening of organ function, and worsening of systemic inflammation. Our infusion team will review your relevant medical history, monitor you during treatment, and prioritize your safety at every visit.

Interactions with Other Medications

Before starting Orencia treatment, it’s crucial to let your ordering provider know about all the medications, supplements, and herbal products you’re taking.

Orencia should not be used in combination with other biologic drugs, as this can increase your risk of serious infections. Additionally, live vaccines are not recommended while you’re on abatacept, since your immune system’s response may be weakened, making the vaccine less effective. Always check with your provider before receiving any new vaccines or starting herbal supplements or other medications to ensure safety and efficacy.
